3. The healthcare institution has to present documentation of at least one certified specialty doctors in each clinic or two or more years of doctor training (for Chinese medicine practitioners).
4. A simple floor plan of the building.
5. Building Use Permit.
6. Original and Photocopy of Healthcare Personnel Certificate.
7. Photocopy of National ID.
8. Medical Establishment Basic Information Form.
9. Photocopy of the Lease.
