Dietitian Practice License Application
Required Documents：
1. Front and back copy, original copy (to be returned after inspection) of dietitian certificate.
2. Health examination form, proof of membership in association.
3. Front and back copy, original copy (to be returned after inspection) of Taiwan ID
4. Three 1-inch half-length photos taken in recent months (indicate name and birth date on back).
5. Documents of applicant.
6. 2 copies of dietitian chart (for first-time applicants).
7. License fee: NT$1,300
