 Required Documents:
1.Establishment, alteration of provided services, person-in-charge, or venue; the bureau accepts application for relevant guidance and verification of facilities.
2.Two copies of the personnel health examination sheets (must include items such as Tuberculosis, Hepatitis A, and skin diseases on the arms, etc.).
3.A photocopy of the occupational technical certificate of relevant personnel.

Application forwarded from the Tax Revenue Service to the Industry and Commerce Section of the Construction Bureau, and the Pharmaceutical and Food Safety of the Health Bureau for processing and confirmation
